Don't Mess with These Menopause Experts! When The Lancet, one of the oldest, most respected medical journals, published a series of articles saying that “menopause had been over-medicalized”, that many symptoms were “just in her head” and that overall, women should navigate menopause “without the need for medical treatments” a group of furious menopause experts went ballistic.  In this episode, Dr. Mary Claire Haver and I tell you why there was such a huge backlash from hundreds of menopause experts, and how we took action.  Dr. Haver and I address 4 specific statements from the Lancet article:  “Most women navigate menopause without the need for medical treatments.” “During the menopausal transition, women should “challenge self-critical beliefs, which can…make [hot] flushes worse.” “Based on scarce data, we found no compelling evidence that risk of anxiety, bipolar disorder, or psychosis is universally elevated over the menopause transition.”  “Over-medicalization of menopause can lead to disempowerment and over-treatment.”   Dr. Streicher’s New Book Come Again! Estrogen is not only good for your brain, your bladder, and your bones, estrogen is also good for your blood vessels. And a lack of estrogen is one of the reasons, that heart disease spikes in women post menopause. Hormone therapy, if started within ten years of the menopause transition, appears to be heart protective. Gynecologists have the unique position that it is appropriate to ask personal, invasive, and otherwise inappropriate questions. One question that is standard when you go to the gynecologist is “Are you sexually active? “This seems like a reasonable question, but is, completely meaningless. Menopause experts rarely see patients with no medical issues. We see the patient with multiple medical problems and concerns.  In this episode, I bring in menopause expert Dr. Risa Kagan to collaborate on options for a complex patient.  My name is Beth, and I am 51 years old.  I’ve been flashing for 2 years with no end in sight.  I really want to try estrogen not just to help my hot flashes, but also because my mom had osteoporosis. I don’t have osteoporosis yet, but I’m worried since I’ve been told I have low bone mass.  My doctor won’t give me estrogen because my grandmother died from breast cancer. She wants me to take raloxifene to protect my bones and my breast, but that won’t help my flashes. I also would rather not take anything with progesterone since when I took it years ago to help with irregular periods, I got moody and bloated. I need some help here. “ *Options discussed include: Raloxifene (Evista™) A Selective Estrogen Receptor Modulator (SERM) and estrogen together Paroxitene (Brisdelle™) Gabapentin Fezolinetant (Veozah™) Fezolinetant and raloxifene together Estrogen and vaginal progestogen gel (Crinone™) Estrogen and a Progestogen IUD Unopposed estrogen Conjugated equine estrogen (Premarin™) and bazodoxifene (Duavee™) *I have no financial relationship with any of the companies that sell these drugs For more information on this topic:  Episode 2:Think Your Hot Flashes Can't Kill You? In the 1960s, under the leadership of Helen Gurley Brown, Cosmo morphed from a “proper ladies' magazine” to a much racier version that was thought of as being a sex manual disguised as a woman’s magazine. The target audience was the young working woman. Women in their 60s were not reading it and were certainly not buying subscriptions for their daughters or granddaughters.  And here we are, Cosmo has just published a groundbreaking, article titled Sex after 60 based on research done in collaboration with the renowned Kinsey Institute of Sexual Research. The Kinsey research team, which I was part of, provided the questions which were ultimately answered by over 3000 women, and then analyzed the data.  In this episode, I have a conversation with sex therapist Dr. Rachel Zar, who was quoted extensively in the article, and who is, as most of you know by now, my daughter.  This episode is based on an article that was published in the January issue of Cosmopolitan Magazine.  https://www.cosmopolitan.com/interactive/a45893760/sex-after-60/ For more information on this topic:  Episode 14: Need to Know Info About the Penis in Your Bed with Dr. Rachel Rubin Episode 16  Minimizing the Monotony of Monogamy with Dr. Alexandra Solomon Episode50 Dating in a Digital World-The Kinsey Report on with Dr. Justin Garcia  Episode 54 What Happens During Sex Therapy with Rachel Zar  Episode 89  Faking Orgasm with Rachel Zar  Slip Sliding Away: Turning Back the Clock on Your Vagina-A gynecologist’s guide to eliminating post-menopause dryness and pain Rachel Zar PhD, LMFT, CST is a couple and family therapist and an AASECT-certified sex therapist. While many women are aware that hormone therapy is good for the bones, it is usually thought of as a side benefit. You know, women take hormone therapy because they are flashing or can’t sleep and then are told, are told, by the way ..estrogen will also help prevent bone loss.  In the United States, there are 10 million women with OSTEOPOROSIS and 43 million women with OSTEOPENIA. Most of these women have no idea that they are at risk for a potentially life-changing or even fatal fracture and also have no idea that hormone therapy is a proven strategy to treat low bone mass and prevent osteoporosis. Today, I am joined by bone guru Dr. Kristi DeSapri to talk about the role of hormone therapy when it comes to low bone mass and osteoporosis.   In response to questions that came in on social media, we address: Is hormone therapy preventive against further bone loss or will it also help build bone?  Is the same DOSE that prevents HOT FLASHES also going to protect your bones?  When it comes to bone health, does it matter if you use TRANSDERMAL  or ORAL estrogen? What about TESTOSTERONE?  Can you use hormone therapy instead of bone-building drugs? Is it ever TOO LATE to start hormone therapy to help your bones?  And so much more…  For more information on this topic:  Episode 2:  Think Your Hot Flashes Can't Kill You? Think Again! Jill Wine-Banks may be known to you as the Watergate Special Prosecutor who famously cross-examined Rose Mary Woods, President Nixon’s secretary, about the 18-minute gap in the Watergate tapes.  In addition to multiple other high-profile jobs (scroll down for the list!) decades later, she is now an MSNBC Legal Analyst. In addition, she is the author of The Watergate Girl: My Fight for Truth and Justice Against a Criminal President.  Jill is also one of the hosts of two hit podcasts- Sisters in Law, and iGen Politics. But, this is not a political podcast. I usually talk about hot flashes and hormones. So, why is Jill Wine-Banks my guest today?  Because Jill Wine-Banks is the embodiment of a woman who in her 70s started multiple new chapters and at age 80, has an entirely new career.  In today’s episode, we chat about: What it was like to be one of the few women in a male-dominated and sexist profession The impact of a sexless, loveless marriage How she came to be the Watergate Special Prosecutor and how that changed her life Her experience with hot flashes and hormone therapy  The secret to staying relevant and purposeful no matter what your age.  And so much more….                                                          Jill Wine-Banks https://www.jillwinebanks.com/ The Watergate Girl: My Fight for Truth and Justice Against a Criminal President  Podcasts: #Sisters in law iGen Politics Podcast Social Media: @jillwinebanks Jill’s Jobs First woman to serve as an organized crime prosecutor at the US Department of Justice Watergate Special Prosecutor  Named General Counsel of US Army by President Carter Eliminated the Woman’s Army Corps and integrated women into the Regular Army and West Point and basic training.  A recurrent theme in my podcasts is that most medical students and residents do not get training in sexual medicine and menopause, which is why most doctors are not experts. And why so many women get frustrated that finding solutions for their menopause symptoms is too often a do-it-yourself research project. But today, I am joined by someone who is passionate and dedicated to educating the doctors of tomorrow.    This episode was initially aired on Sex Med for Sex Ed, hosted by Dr. Terry Gibbs, an OB-GYN and the founder of The Ohio Sexual Health Collaborative, a multi-disciplinarian organization of sexual medicine clinicians.  • My paper on dyspareunia (link- there may be a paywall)  • Things that cause painful sex beyond GSM • And why treating painful sex  is often not treated appropriately  • Getting the most out of a doctor’s visit • Use of CBD to help with painful sex • Laser treatment for treatment of vaginal dryness • Post-menopause orgasmic dysfunction • Medical misinformation • Why The Women’s Health Initiative (WHI) was problematic.  • Why the word “provider” is problematic and insulting • Update on my menopause retreats Further information: Link to dyspareunia paper https://pubmed.ncbi.nlm.nih.gov/37040586/ Streicher LF. Consider the “cycle of sex” throughout our lives. In your twenties, it’s all about sex, all the time. Then there’s the baby-making years, where sex has a purpose. And before you know it, the bedroom is a place where all you want to do is get a decent night’s sleep.  The years go by, and suddenly it occurs to you that you can’t remember the last time you had sex. And if you try to get things going your brain and your body don’t necessarily cooperate.  And then things get even more complex during perimenopause and menopause, when, in addition to dealing with plummeting hormones, insomnia, hot flashes. medications, stress, you may also be dealing with a rotten relationship, or no relationship.  So, what about taking a medication to give a lift to a lagging libido? In a perfect world, an alert would pop up on your phone announcing, “You are now in perimenopause,” followed at the appropriate time by “You are now leaving perimenopause and entering post menopause.” But it’s not a perfect world, and unless you are thrust into menopause because of ovary removal or chemotherapy, in many, if not most cases, it’s hard to know what your ovaries are doing. Menopause is defined as starting when you haven't had a period in 12 months. And while that’s a very specific definition, that definition is only useful in retrospect. And the 12 months without a period definition is totally meaningless for the millions of women who don’t get periods because of maybe a hysterectomy or a uterine ablation. I start this segment by discussing  (and reading) my recent editorial on this topic in the journal, Menopause. I then go on to address:  Why the current definition is problematic, confusing and impacts research How to know you are perimenopause or post menopause if you DON’T GET PERIODS Post Hysterectomy Post Endometrial Ablation With IUD Use If You Are Taking Birth Control Pills Starting hormone therapy when not sure what stage of menopause you are in Starting hormone therapy if you still getting periods but are having symptoms My Editorial: Is it Time to Rethink How Menopause Is Defined (There is a paywall so non-subscribers will not be able to access the article) Streicher, Lauren MD, MSCP.  If you think you’re having a hard time navigating vaginal dryness, painful sex, insomnia, and hot flashes, I can guarantee that if there is a man in your life, he is also having a hard time. More likely than not, he is completely in the dark about why you are having these symptoms and what he can do to help. In this episode, I sit down with Rachel Hughes, the host of Perimenopause WTF to discuss five things guys need to know about menopause. This segment was originally aired during a live webinar on The Perry Channel.  Rachel and I also take questions from the audience. And no surprise, we came up with more than five things that men need to know to support the women in their lives.    When and How to use lubricant. Understand that a vibrator is not a reflection of your inability to please her. It’s a reflection of your ability to please her. What’s with the hot flashes?  Her lack of libido isn’t about you. It’s about the fact that she hasn’t slept, sex hurts like hell and she isn’t feeling great about her body changes. There is no expiration date on a vagina. How to start the conversation. The number one complaint during perimenopause and menopause is insomnia. When a woman goes to her doctor and asks for help, she is rarely if ever, told that cannabis may be the answer. When someone is diagnosed with uterine cancer, in most cases, their uterus announces there is a problem by bleeding when it is not supposed to, or bleeding too heavily. Breast cancer, in most cases makes itself known by a lump, a nipple discharge or an abnormality on a mammogram. But ovarian cancer is not so accommodating and, in many cases, does not make its presence known until it has already moved well beyond the ovaries. My guest today is Dr. Shieva Ghofrany a practicing Ob-Gyn who was diagnosed with stage 2 ovarian cancer at age 46. She is going to share her story and talk about early signs, symptoms, and detection of ovarian cancer in women who are at average risk.
